There is a 7 day waiting period for benefit eligibility and the employee must use up their accumulated sick leave before receiving payment under the Short Term Disability Plan.
The amount of short-term disability benefit you receive is based on your choice of a weekly benefit, but cannot exceed 70% of your basic weekly salary or $ 1,000 per week. There are two options available to you regarding the weekly income allowance.
You can begin receiving short-term disability benefits once your accumulated medical leave has been used up and a completed application form has been received and approved by Cigna.
For pregnancy, STDs are limited to 6 weeks for normal childbirth and 8 weeks for cesarean section.

- 50% plan – Monthly salary / 100 x 0.297

The level of coverage you choose cannot exceed 70% of your gross weekly salary amount or $ 1,000 per week.
Example: If an employee earns $ 35,000 per year or $ 2,916.67 per month, the cost of delivering the plan at 70% with a weekly maximum of $ 1,000 would be $ 11.54 per month or $ 5.77 by paycheck.
The level of coverage you choose cannot exceed 50% of your gross weekly salary amount or $ 1,000 per week.
Example: If an employee earns $ 35,000 per year or $ 2,916.67 per month, the cost of delivering the plan at 50% with a weekly maximum of $ 1,000 would be $ 8.66 per month or $ 4.33 by paycheck.

There are several different weekly benefit options to choose from. However, the maximum benefit payment you choose cannot exceed 70% of the employee’s basic weekly earnings or $ 1,000 per week. Your bonus is based on your salary.
If the disability is caused by accidental injury, there is no waiting period. If the disability is caused by illness, there is a 7 day waiting period before benefits are paid. The maximum number of weeks during which benefits can be paid is 13 weeks.
If you exhaust your Short Term Disability and need to apply for Long Term Disability, the City of Scottsdale will pay GAP pay until you reach this 6 month waiting period. GAP pays 60% of your salary from the 90th day of disability to the 180th day of disability at no cost to you. All accumulated leave must be used up before GAP is paid.
You must apply for long term disability or you will not be paid.
LONG TERM DISABILITY
If you contribute to the Arizona state pension system, you contribute 0.14% of your salary for long-term disability. ASRS will process and approve or deny your application for Long Term Disability benefits.
If you contribute to the Public Security Staff Pension Plan, CIGNA will process and approve or deny your application for long term disability benefits.
